오늘은 유두 재건에 대해 알아보겠습니다.
유두는 젖꼭지라고도 하며 평균 9mm 정도 튀어나와 있지만 그 높이는 개인차가 큽니다.
유륜은 유두 주변에 둥글게 위치하며 색깔은 분홍색에서 짙은 갈색까지 다양합니다.
유륜에는 몽고메리 결절(Montgomery tubercles) 이 있어 표면이 매끄럽지 않고 울퉁불퉁합니다.
이 중 유두는 두가지 방법으로 재건이 가능합니다.
유두를 만들고자 하는 부위 주변의 조직을 모아서 튀어나오게 만들어 봉합해주는 피판술과 반대편 유두의 일부를 잘라 붙이는 조직 이식방법입니다.
반대편 유두가 너무 큰 경우, 반대편 유두 크기를 줄이면서 유두를 재건하려면 조직이식방법이 적합합니다.
그렇지 않다면 피판술이 적합하겠습니다.
두가지 방법 모두 시간이 지나면서 수술 직후보다 크기가 줄어들기 때문에 크기를 크게 만들어야 합니다.
우리나라 여성의 경우 유두 크기가 많이 크지 않기 때문에 피판술을 이용하여 유두를 만드는 경우가 많습니다.
저희 병원 수술실 직원들이 볼 때마다 신기해 하는 피판술을 설명하는 그림입니다.
피판술을 할때는 크기 감소를 줄이기 위해 열이 발생하는 수술기구를 사용하지 않고, 세심한 디자인을 통해 새로 만든 유두에 충분한 혈액공급이 되도록 하여야 합니다.
